Targeted Case Management is a service that aims to eliminate barriers to treatment by addressing the social, emotional, educational, and vocational needs of clients. Targeted Case Managers work closely with the client and their family to assist in navigating mental health care, while advocating for their needs across domains of functioning.

Case Managers work with the client to provide care plans that include individualized goals and objectives aimed to support the entire treatment team and wrap services around the client to be successful in the mental health treatment journey.
